3. If Private Doctor we need: 2 doctor blank letterhead (With signature & Stamp)
4. If Doctor we need: P.M.D.C. certificate
5. If Dealer we need: 2 blank dealer letterhead (With signature & Stamp)
6. If Dealer we need: Dealership certificate
7. 4 picture white back ground size (48 X 33mm) matte finish paper without glasses, scarf, cap & ear must be visible (less than 3 months old)
